CKC Entry Forms

If anyone is interested, I have created some CKC entry forms using Adobe that have pull down menu's and that will fill in other area once you enter the info once, i.e. Owner and the handler are the same person and you are going to be attending both days, you only need to enter the info once. I have also created ones where the handler/owner and dog are all different.
Anyways, I created them for my own ease of use, but you are more than welcome to them if you think they would be useful. Just pm me.

Mike I would love to see that as we enter 3-5 dogs per weekend and writting them out is a pain. generally we have them copied then fill in the date and cat when we need to send them

Great job on those forms, I think it needs to be sent to CKC so they can post it on the website for everyone to use!!

I don't mind you fowarding them at all. I just want to make sure that the forms are as good as they can be before I pass them on. I was hoping you guys could give me some input on changes you would like.

Ahh, well the date formay is dd-mmm-yyyy, yet it changes the yyyy to two digits, so you may as well just make it dd-mmm-yyy

Alsounder the "official canadian kennel club" line, a lot of clubs put their name in, so maybe make that a box, so that you know WHICH premium it is your sending? not saying that's the right thing to do, just giving you suggestions...

I think it'd be a good idea if it defaulted to Female, since that's what I have, as well as maybe had my information in it as a "sample" so people know what to type! ;)

Also you may want to consider removing QC from the province list, since really they'll have their own kennel club someday ;)
